We are very excited to have you join our medical family.
Here are some points to take note of for your first appointment.

  • Make sure that you have verbally confirmed your new patient appointment with our office staff. If you have not confirmed your appointment, it may be cancelled.

  • Make sure that you have called your insurance company and changed Dr. Jhaver to your Primary Care Physician. Your insurance card should have Dr. Jhaver’s name on it to ensure your insurance company does not bill your visit as an out of network visit.

  • You will need to bring your insurance card, picture ID and copay with you to your first appointment.

  • Arrive 15 minutes early to your appointment to complete paperwork. To make the sign in process easier please feel free to download and complete the New Patient Forms prior to your appointment.

  • Please bring an up to date list with the names, doses and frequency of your medications.

  • Please bring any recent medical information you may have that you feel the doctor may need such as labs, imaging reports or consultant notes.

  • Please bring the names and contact information of any physicians you would like Dr. Jhaver to request your medical records from.   You can also download and complete the Medical Records Request Form prior to your appointment.

  • Your first appointment will always be billed as a new patient appointment.

  • Please note this office does not refill long term controlled substances. We are more than happy to refer you to the appropriate specialty to ensure your refills of these medications are properly addressed.
